| Social Worker II is responsible for providing direct psychosocial interventions and related counseling to patients and their families, including crisis intervention. Provides group therapy to outpatient population/ Performs psychosocial assessments and interventions utilizing psychosocial/social work methods, based on patient’s assessed needs. The social worker provides community resources and referrals. Provides consultation as needed to physicians and other members of the treatment team regarding the continued needs of the patient. Is the key person to provide the facilitation, implementation and coordination of the patient’s discharge plans. Is knowledgeable about the legal aspects of the patient’s care. Enters social work services notes in the patient records. Participates in treatment planning. Reports to the Director of Outpatient Behavioral Health. This position may provide direct patient consultation services and group therapy to adult and geriatric populations.
Qualifications: BCLS. CPI/AB508 or other Assault Behavior Prevention Intervention class completion desirable but will train within 60 days of hire. Master’s Degree in Social Work or related field, eligible for license with the state of California as Marriage Family and Child Counselor Intern or Clinical Social Worker Associate.
