Position Summary:

The Care Coordinator is responsible for providing high quality outpatient therapeutic or residentially based services and/or case management to at risk youth and families who may be experiencing barriers to receiving office-based therapeutic services. Services to assigned youth and families occur in the community in a variety of settings (e.g. home, or school setting).
The position is also responsible for case management, administrative documentation, and participating in trainings as assigned. Along with the rest of the team, the Care Coordinator is active in supporting and implementing wraparound philosophy and its individualized, strengths-based, culturally competent, family-centered approach.

Requirements:

      1. Master’s degree in Social Work or Counseling from an accredited college or university, Required.
      2. Registered intern or associate with the Board of Behavioral Sciences, or able to obtain registered status within first 6 months after hire, Required. Registered PCC intern (PCCI)* with relevant experience with target population eligible in San Diego/Alameda Counties. (If working in a program licensed by Community Care Licensing must meet minimum training and experience requirements.)
      3. License (LCSW or MFT) Preferred (in some cases required); or, in San Diego/ Alameda County Licensed Professional Clinical Counselors (LPCC)*.
      4. Clean driving record; current CA driver’s license Required, or ability to obtain within 10 days after hired.
      5. For staff delivering behavioral services (e.g., TBS), experience with or knowledge of designing, supervising, monitoring, changing, and discontinuing individualized behavioral interventions required.
      6. At least one year post-graduate school experience with program target population preferred (Example: intellectually disabled, homeless, teens or young adults, SED); at least one year experience with program modality (example: school-based, residential, mobile/community-based, assertive community treatment, wraparound, etc.) Preferred.
      7. Preferred: demonstrated ability to assess, triage and organize work.
      8. Preferred: Uses a strength-based, culturally competent approach to supervision, participant and community service.
      9. Preferred: Experience with relevant evidence-based practices (e.g., CBT, DBT, trauma-informed approaches)
      10. Preferred: Demonstrated ability to work effectively with diverse families and communities.
      11. Preferred: Has strong behavioral, crisis management and family/individual/group therapy skills.
      12. Preferred (sometimes Required): Fluency in prevailing language(s) of the community. Bilingual Preferred.
      13. Must have access to dependable transportation and personal car insurance, meeting minimum liability coverage required for position if driving for work or transporting participants.
      (*) For LPCC/PCCI’s that County specific eligibility must be verified by the hiring manager including coursework, experience, and CEUs, as required.
