Description
Duties: Therapist is responsible for monitoring the delivery and coordination of a broad range of services to clients. Conducts outreach and engagement to homeless-chronically mentally ill adults, completes intake process, develops and monitors individual treatment plans, conducts crisis interventions, and ensures those clients are receiving an integrated program of services that provides a high probability of treatment success.
The therapist makes clinical evaluations, conducts individual and group psycho-therapy, provides targeted case management, and participates in rotation of 24 hour crisis line. Works with outside agencies, follows up on referrals; builds collaborative agreements with outside agencies. The therapist ensures that all services provided are documented and recorded in a timely manner.
MINIUM QUALIFICATIONS: ACSW, LCSW, MFT, MFTI, or MFCC in clinical psychology. Five years experience in social service or drug treatment; knowledge and understanding of the Hispanic community problems, Mental Health services and treatment; and bilingual/bicultural.
