We currently have an opening for a regular full-time Case Manager at our South Bay Guidance Wellness & Recovery Center. The duration for this position is up to three (3) months.
South Bay Guidance Wellness & Recovery Center offers a wide range of outpatient mental health services to the culturally diverse adult population of San Diego’s South Region. They also provide initial psychosocial assessments, psychiatric and medication evaluations, medication support services, limited individual supportive counseling, group counseling, care coordination, information, referrals and linkage, crisis intervention, and psychosocial rehabilitation activities. South Bay Guidance provides these services under contract with the County of San Diego Mental Health Services and the Health and Human Services Agency to individuals 18 years of age and older.
GENERAL DESCRIPTION
The Full Service Partnership Case Manager/Rehab Worker provides enhanced services to adults and their families utilizing the Strength Based Case Management model. This position promotes healthy development and positive outcomes for families through utilizing case management and rehabilitative strategies and providing direct support services to enhance life skills, and problem solving capabilities. The Case Manager assists clients and families build upon natural support systems and links individuals to other service agencies as appropriate for the family. Many of the services will be provided in the homes and other community settings. A high level of confidentiality is required for this position.
Duties include, but are not limited to, the following:
· Identify Client and family case management needs
· Connecting every client/family to a medical home
· Providing education regarding mental health
· Providing education regarding physical health
· Linkage and referral services as appropriate
· Advocacy
· Other supportive services to assist client to meet their treatment goals
· Provide groups for parents to enhance parenting skills
· Work cooperatively with the treatment team at SBGWRC
· Attend all meeting related to this position
· Document in the electronic and paper record as per CRF and County policies
QUALIFICATIONS / SKILLS REQUIRED
The ideal candidate possesses a Master’s degree or higher in a mental health-related field and is licensed or close to licensure or Registered Intern with the BBS. Experience working with TAY clients 18-25, Adults 26-59, and Older Adults 60 plus years old and knowledge of County Health and Human Services requirements including documentation.
Microsoft Office software experience is also required in this position.
A valid California driver’s license in good standing and eligibility to be an approved CRF driver is required to fulfill the essential functions of this position. Determination of driver eligibility is based on criteria established by the County of San Diego, CRF’s automobile insurance carrier and the California DMV. Eligibility must be maintained through the duration of incumbency. In addition, proof of insurance is required prior to driving personal vehicles on company business.
