Description

The Family Facilitator coordinates and provides leadership in the development and implementation of the service plans, provides direct services to children and their families and facilities the integration and coordination of services.  Assumes responsibility for the Plan of Care management, implementation of services / coordination of resources (monitoring the Child and Family Plan expenses / budget and securing community team approval for overall budgeted plans); submits required billing forms and progress reports on a timely basis. Serves as a role model / advocate for families involved in the wraparound process and ensures timely and appropriate communication and collaboration with our partner agencies and community resources.

Qualifications

Education/Experience:

Master’s degree is required w/ at least two years experience working with SED/high risk children, adolescents, and their families with in a mental health setting. Must be Marriage & Family Therapy (MFT) Intern or MSW Intern. Qualified candidates must have strong case management/facilitation skills and the ability to engage parents as partners in the treatment of their child.

Individual must also demonstrate leadership qualities and have the ability to successfully work with families from diverse ethnic and cultural groups.  Candidate must also and have experience with crisis management.  Bilingual preferred.
