The Jane Westin Center is growing…with brand new positions and shifts available!
As of January 1, 2019, the Jane Westin Center will be open 8:00 am – 12:00 am, 365 days/year!
Program Overview:
Located in downtown San Diego, CA, the Jane Westin Center is an adult outpatient walk-in clinic which provides a variety of intervention services to individuals in crisis. We serve individuals with serious mental illness and co-occurring substance abuse disorders.
Our services include initial psychosocial assessments, psychiatric and medication evaluations, crisis intervention and linking clients with ongoing community-based services.
Typical Duties of the Assessment and Discharge Specialist
*Provides screening, clinical assessments, case management and outreach for outpatient program clients diagnosed with severe mental illness and may be dually diagnosed with a substance abuse disorder
*Conducts collateral interviews, develops individual service plans and participates in interdisciplinary treatment planning
*Provides crisis intervention as appropriate
*Coordinates with the multidisciplinary team regarding discharge planning in progress
*Collaborates with nursing staff to arrange continuing care medication maintenance
*Assists clients with obtaining treatment benefits and other community-based supports to facilitate recovery and improvement
Qualifications/Skills Requirement
*Must be licensed or licensed eligible (Waivered/Registered Psychologist, AMFT, ASW or APCC)
*At least 2 years of experience with working with individuals who have a severe and persistent mental illness, as well as co-occurring disorders required
*Those with experience in a walk-in outpatient-based mental health program encouraged to apply
*Proficiency in MS Office applications, specifically Word and Excel required
*Must be able to work in a fast-paced and collaborative environment
*Able to pass criminal/background clearance
