Our dedicated teams work collaboratively and in partnership with family members, school administrators and faculty, county social workers, case managers, probations officers and other agencies.
Job Summary:
This position plans, coordinates and implements services for families. In-Home Outreach Counselors (IHOC) I/II provide case management, crisis intervention, linkage services and advocacy. He/She provides in-home outreach counseling services to families based on approved service plan. In collaboration with the family and DCFS Staff, the IHOC’s conducts psychosocial assessments, develops comprehensive treatment plans, and arranges for activities and services while monitoring the client’s progress toward a court ordered and non-court ordered treatment/case plan goals. In-Home Outreach Counselors documents and communicates clients’ progress, strengths and needs in a timely and accurate manner through all necessary reports. He/She attends various planning, training and coordination meetings conducted by DCFS and others; Providing linkage to needed resources/services and following-up to ensure successful outcome.
Job Duties:
Provide in home counseling to clients and their families in accordance with appropriate level of service.
Provides crisis intervention, advocacy and support services on a weekly basis.
Provide case management services, including but not limited to linking clients to auxiliary services and assisting clients and their families with accessing appropriate resources.
Interacts and communicates with clients, their families, representatives of referring agencies, clinical and non-clinical staff and other relevant parties (in-house and external) to promote rehabilitation and amelioration of problems which interfere with productive social and emotional daily living.
Conducts upfront assessment within 24 hours of receiving referral, using the BSAP or other county-approved instruments.
Minimum Qualifications:
IHOC II: Master’s degree in Mental Health, Human Services, or closely related field and one (1) year experience in mental health field, and direct service experience with severely emotionally disturbed families/children (experience with high risk families/children). Licensed MFT or LCSW, or Registered MFT or MSW Internship in California preferred. \
IHOC I: Bachelor’s degree in a Mental Health, Human Services, or closely related field and two (2) years’ experience in mental health field, and direct service experience with severely emotionally disturbed families/children (experience with high risk families/children).
Valid California Driver’s License.
