Program Summary:

JJDTP is a Full Service Partnership for probation-connected youth and their families, designed to enable youth with a Serious Emotional Disturbance and current involvement in the juvenile justice system to remain in their homes, schools and communities by providing a comprehensive array of mental health services and supports. Youth are referred between the ages of 13 and 17 years and may participate up to age 26. Goals include stabilizing placements, reduce homelessness, support academic participation, increase vocational or employment opportunities, reduce psychiatric hospitalizations and detention or incarceration.

Job Responsibilities:

  • Prepare and present clinical and/or case management documentation for review by treatment team or management; provide on-going consultation and systematic support for treatment team in the implementation of treatment plans for individual clients and families
  • Develop and administer long or short-term treatment plans requiring comprehensive professional knowledge consistent with assigned programs mission and guidelines; may include clinical treatment plans and may determine appropriate method of treatment intervention including behavior modification
  • Perform home, school, office and work visits to observe and assess the client in the appropriate setting(s); plan and participate in activities; listen to the client and/or family in a supportive and goal directed manner; empower the client and family; may perform on call duties
  • Participate in and/or conduct family and individual client therapy to affect positive rehabilitation, solutions, and/or life improvements
  • Perform case management duties including treatment plan monitoring, advocacy, referral and linkage to other needed services and crisis interventions; establish and maintain casework records and documents in accordance with program requirements and procedures; and prepare relevant correspondence, reports, and documentation
  • Direct, coordinate, and participate in the development of client goals; work with client, family and team to define goals, specify strengths, and identify client/family service needs; direct and participate in the writing of program specific treatment plans and client/family plan(s)
  • Ensure confidentiality of client records and files, prepare and/or oversee the participation of required reports and summaries; monitor clinical record maintenance due dates; perform and/or coordinate client record and file reviews; comply with utilization reviews
  • Supervise, train, and monitor assigned subordinate staff members, mentors, and/or volunteers; participate in providing performance feedback; conduct treatment plan and/or staff meetings as appropriate
  • Meet established billing goals; track productivity levels on a consistent basis; manage self to achieve established productivity goals and modify behaviors accordingly
  • Maintain awareness of and participation in external professional environment and resources to ensure the highest level of professional standards are applied to service delivery by self and other team members
  • Demonstrates concern and courtesy to clients, families, vendors, other staff members, and the general public by giving appropriate information, advice, and assistance
  • Demonstrates the skill to show respect and understanding of client and family, display empathy and maintain effective work relationships with those contacted in the performance of required duties
  • Proficient with Outlook, Microsoft Word, Excel and PowerPoint
  • Maintain a variety of online recordkeeping and documentation processes and systems for assigned clinical program; ensure all relevant confidentiality and privacy requirements are met

Minimum Qualifications:

Must have at minimum:

  • Possess a Masters Degree from an accredited university or college with major course work in social work, psychology, and counseling or a closely related field
    • PLUS possess a current registration with the Board of Behavioral Sciences (associate OR valid LCSW, LPCC or LMFT license, plus additional experience)
  • Must have excellent organizational, analytical, communications, and counseling skills.
  • Must be able to empower people with a strength-based approach, prioritize responsibilities, and build rapport with a wide variety of individuals.
  • Candidates in their last semester of graduate school may be considered. If the degree is complete, must become registered with the CA Board of Behavioral Sciences or Board of Psychology
