POSITION TITLE:  (In Home Outreach Clinician), Family Preservation Program

DUTIES AND RESPONSIBILITIES:

Under the direction of the Program Director for Family Services, the In Home Outreach Clinician will:

·         Facilitate a case-management system and evaluation of service needs for families referred through the Department of Children and Family Services to the Family Preservation Network.

·         Provide direct in-home outreach counseling to Family Preservation Program client families.

·         Arrange and attend the Multidisciplinary Case Planning Committee Meeting for each family in treatment.

·         Provide parenting skills training.

·         Work with the Department of Children and Family Services CSW as needed to assist in accomplishing case plan goals and outcomes for Family Preservation clients.

·         Provide appropriate referrals for family services needs and maintain follow-up on progress.

·         Maintain accurate and timely documentation of all job related activities.

·         Attend assigned meetings and trainings.

·         Other duties as assigned.

REQUIREMENTS:

·         Master’s Degree in social work or counseling with appropriate state license in social work or marriage, family and child counseling.

·         Ability to work a flexible schedule, including some weekends and evenings.

·         Permanent U.S. residency or citizenship.

·         Valid California Driver’s license, current auto insurance and good driving record.

·         Ability to pass background check and criminal clearance.

PREFERRED QUALIFICATIONS:

·         Bilingual in English and Spanish

·         Three years experience in conducting groups and individual counseling with children and families.

·         Experience in working in a community based organization.

·         Knowledge of Southeast Los Angeles County.

·         Trained in facilitating parenting skills groups.

Perks & Benefits:

2 weeks vacation, accruals start day one. After 3 years of employment, (3) weeks of vacation is accrued. After (7) years of employment, vacation accruals increases to (4) weeks per year.
2.4 weeks of sick time (12 days per year)sick accruals start day one.
Dental is PPO through Humana and the expense to employee is $1 per pay period$2 per month.
Health Insurance – We have several HMO Options including Kaiser and Health Net; cost to employee ($70 per month) there is also a PPO option.
Holiday schedule is 17 ½ days per year
