We currently have an opening for a regular, full-time Discharge Planner at the Jane Westin Center .

Jane Westin Center provides urgent, psychosocial assessment and psychiatric services in an outpatient setting to the culturally diverse adult population of San Diego’s Central Region. The emphasis of our treatment is on stabilization of the client with a psycho-social rehabilitation orientation, then linkage to on-going care in the community. Respect for the client is a core value, and our staff work closely with all of our clients to develop individualized discharge plans that focus on their strengths, needs and abilities. We encourage the participation of family members in the recovery process. Our community holds a wealth of knowledge and experience, and including additional people in the support systems of our clients holds the benefits of improved health and stability.

General Description

The Discharge Planner is responsible for transition planning and connecting clients to on-going care in the community. Works closely with the staff psychiatrists to determine the needs of the client for on-going services and the level of services needed. Provides training to staff regarding community resources, service to clients, and quality representation of the program to the community. There is an expectation that this position requires a large quantity of very high quality work. Strong communication skills are a must in this high paced environment. Investigates resources in the community available to discharging persons served and maintains an up-to-date list of community resources. Intervenes, when necessary, with agencies in the community to facilitate timely placement or appointments. When needed is available for crisis intervention and assessments, or other duties as assigned by the Program Director.

Qualification / Skills Required

The ideal candidate has Master’s in Psychology, Social Work or related field with minimum 2 years related work experience; or Licensed or Waivered PsyD, MFT, LCSW, or Registered ASW, MFT intern. Experience with a walk-in model based program preferred. Training with Anasazi Assessments and San Diego County documentation strongly preferred.

Experience working with clients with persistent and serious mental illness in a high volume/stress environment would be an advantage.
