Position Title: Experience Hope Clinical Case Manager

Area/Program Focus: Mental Health and Violence Prevention Services

Position Summary: This position has primary responsibility for providing clinical case management services to CCEB’s trauma treatment program for youth, Experience Hope for Teens, which provides culturally-competent, trauma-informed mental health services to West Contra Costa Unified School District students and their families. Clinical Case Managers will deliver evidence-based trauma treatment to youth and families and be required to maintain fidelity to program and treatment models.

Reports to: Clinical Supervisor

Overall Leadership:

  • Leads on individual activity with clients, developing specialized interventions as needed

Essential Responsibilities:

  • Learn, adhere to, and deliver evidence-based treatment models
  • Establish and maintain effective working relationships with school personnel, parents, and community members.
  • Establish and maintain effective working relationships with a variety of clients from diverse cultural backgrounds.
  • Conduct home visits with students and their families
  • Partner with school staff, CBOs, and government agencies on needs of clients and their families.
  • Adhere to agency and program data collection requirements
  • Ensure client service documentation is accurate and completed within 48 hours of client interaction

Qualifications/Requirements:

  • Possesses a master’s degree from an accredited professional school of social work or university preparing students for California licensure as an LCSW, MFT or psychologist.
  • Possesses a license as an LCSW, MFT, or psychologist or be a registered MFT Intern or MSW Associate with the Board of Behavioral Sciences.
  • Knowledge and skills of principles, models, and techniques of intensive clinical case management with a focus on impoverished and violence impacted communities.
  • Knowledge of laws, ethics and regulations governing mental health counseling and crisis management.
  • Knowledge of short-term and long-term counseling. This must include individual counseling with children, as well as family counseling or group counseling.
  • Knowledge of laws and regulations governing counseling sufficient to guide practice with regular supervision.
  • Knowledge of community resources, especially related to trauma informed practice, restorative justice or chronically traumatized communities is highly desirable.
  • Knowledge of Restorative Practices a plus.
  • Ability to drive to and from schools and community meetings in personal vehicle.

  • Willingness to participate and contribute to agency quality improvement activities.
  • Team Player
  • Knowledge of school based mental health counseling is preferred.
  • Valid driver’s license, proof of insurance and access to reliable incurred vehicle
  • Fingerprinting and background check with clearance to work with children and youth
  • Cultural competence: African American, Latino, traumatized populations
  • Willingness to participate in CCEB quality assurance activities as specified by the agency.

  • Ability to use community resources appropriately as part of the counseling process
  • Ability to establish quick rapport with clients and their families often in time of crisis.
  • Ability to interact and communicate with clients and their families who have suffered a major loss or have experienced repetitive traumas and provide therapeutic case management services linking clients to community resources.
  • Demonstrated ability to provide culturally competent case management and mental health support to grieving and traumatized children, youth, and families.
  • Computer skills a must; competency utilizing a client centered database.
  • Ability to produce timely, clear and concise documentation and timely billing.
