JOB SUMMARY

The Wraparound Facilitator/BBS Registered Clinician develops a supportive and helping relationship with high-risk youth and their families that follows the Wraparound philosophy and approach. The Facilitator ensures that clients receive the full spectrum of services in a seamless manner and that they receive the optimal level of care needed to maximize functioning and minimize the need for higher levels of care. The Facilitator is responsible for overseeing the clinical services provided and is responsible for the client’s medical record. They serve as the hub of the team and complete intake, behavioral health and discharge assessments.

RESPONSIBILITIES

  • In collaboration with the youth’s team members and family, serve as the main contact of the Wraparound process, providing team-based services in an in-home and community based environment.
  • Make initial contact with clients and provides strength-based individual and family or group mental health services, in the home or on site, including assessment, treatment planning, Wraparound meetings, case management services, and crisis intervention.
  • Conduct Behavioral Health Assessments and co-creates individualized Client plans with the team and family.
  • Complete Medi-Cal documentation (progress notes, assessments, client plans, and discharge summaries) and billing per County and program standards in the Electronic Health Record.
  • Design, plan, and organize individualized clinical services for each client and family, including Wraparound meetings, individual services, and psycho-education groups.
  • Provide case management services for the client and family in the home or in the community.
  • Transport clients when necessary in own vehicle.
  • Promote and interface with interagency resources as appropriate to caseload and client/family plan. Consult and collaborate with parents, caregivers, schools, referral sources, and other agencies including behavioral health providers, CWS, and Probation.
  • Maintain a flexible schedule according to client needs.

QUALIFICATIONS

  • Active BBS registration as Associate or Licensed MFT, PCC, or Social Work.
  • Master’s degree in social work, counseling, psychology, or related behavioral science.
  • Minimum two years’ experience working in a mental health setting with children and youth with severe emotional disturbances.
  • Considerable knowledge of child development.
  • Fluency in English required; Fluency in Spanish preferred.
  • Valid California driver’s license, reliable transportation and current auto insurance per contract requirements.
