The RA Clinical Case Manager is part of a multidisciplinary team that provides community based mental health services, with an emphasis on skill building and case management to transition age foster and probation youth participating in Side by Side’s Transitional Housing Programs. This includes partnering with youth on their mental health goals, housing, vocational, and educational needs, engaging in positive relationships, and enhancing independent living skills. The RA Clinical Case Manager actively supports and implements a strengths-based, culturally competent, client-centered approach to working with TAY. The RA Clinical Case Manager will share a caseload of 10-13 youth with the Youth Specialist. This exempt position may require work weeks in excess of 40 hours as well as evening and weekend work.

RESPONSIBILITIES:

  • Manage caseload of transition age youth
  • Work on short and long term mental health goals
  • Ensure culturally sensitive and relevant treatment
  • Perform Medi-Cal administrative activities
  • Partner with youth in maintaining relationships with permanent support
  • Coordinate with housing program to achieve treatment and independent living goals
  • Write clinical reports: Progress Notes, Assessments, Treatment Plans and Discharge Summaries
  • Partner with youth in accessing health care, employment, housing, social security benefits, etc.

QUALIFICATIONS:

  • Masters in Social Work or Marriage and Family Therapy or Professional Clinical Counseling from an accredited university or college
  • Registered (AMFT, APCC or ACSW) with California BBS.
  • At least one year post-graduate experience working with adolescents, preferably current and/or former foster youth
  • Licensing fingerprint clearance: California Department of Justice, FBI and CA Child Abuse Index
  • Valid CA driver’s license, dependable personal vehicle for use on the job, insurable by SBS’s insurance
  • Knowledge of foster youth, child development, domestic violence and substance abuse
  • Proficiency in Microsoft Office programs, particularly Word and Excel
