Responsible to provide community outreach, education and staff training on issues specifically related to older adult mental health needs. Carries part time caseload of older adult clients and provides primarily home-based therapy addressing needs of seniors who have mild to moderate mental health issues. The Facilitator is responsible for coordinating Child Family Teams which consist of a Parent Partner, a Youth Support Counselor, the assigned family, friends of the family, and various county partners including Social Workers, Mental Health Workers, and Probation Officers. The Facilitator provides some direct care but primarily ensures that the clients’ needs are fulfilled by the Parent Partner or Youth Support Counselor.
