The Social Worker II is responsible for providing clinical care management to seniors and adults with disabilities and holocaust survivors and their families in a culturally competent and comprehensive manner. Population includes seniors, younger adults with disabilities, and Holocaust survivors and families.
QUALIFICATIONS:
Master’s degree in social work or related field from an accredited school and license eligible. MFT or LCSW preferred.
Bi-lingual English/Russian preferred, knowledge of Hungarian a plus.
Excellent clinical, written and verbal communication skills.
Knowledge of community resources and the aging process.
Experience working with seniors, caregivers and mentally disabled clients.
Knowledge of safety issues when working with at-risk clients.
Computer proficiency in MS Office applications, Outlook and Internet research
Flexibility and ability to work autonomously.
Culturally sensitive and able to work with a multicultural population.
Previous experience and knowledge in working with the holocaust survivor population preferred
Ability to work in a non-judgmental manner and foster positive growth/outcomes.
Able to complete extensive paperwork in a timely manner.
Able to work as part of a team and maintain professional boundaries
RESPONSIBILITIES/ESSENTIAL FUNCTIONS:
Provide social work services for a caseload that primarily includes Holocaust clients, seniors, involving assessment in the home, coordinating and monitoring purchase of services for clients, and implementing a plan of care
Provide intake resources and concrete services
Case management
In-home assessments
Crisis intervention
Responsible for providing psychotherapy and group facilitation
May provide direct services to seniors and families offsite at other JFS locations
Participation in weekly supervision and care plan meetings
Meet with supervisor weekly and prepare monthly client reports to supervisor and in response to government and private funding sources
Compliance with all agency and program documentation requirements.
Contribute to efforts to increase the visibility of the Valley Storefront Senior Center outreach and networking activities as needed
