PROGRAM: ACHIEVING CHANGE TOGETHER (ACT)/PARTNERSHIP FOR FAMILIES (PFF)

SALARY: $48k-$52k (Annually)

SUMMARY:

ACT/PFF is a child abuse prevention program serving high-risk families referred from the Los Angeles County Department of Children and Family Services (DCFS) and pregnant women residing in the South Los Angeles community who have risk factors for child maltreatment.

  • Providing crisis intervention, intake assessment, and individual, family, and group therapy to adults, children and families with a history of mental illness.
  • Plans and administers Family Therapy sessions which may include use of cognitive behavioral interventions as prescribed by the FFT model. Provides a range of services including individual, family therapy, and community based social rehab activities, collateral contacts and case management services.
  • Contact new referrals immediately upon receipt. Performs intake assessments and report back difficulties engaging clients into the program to the Program Manager and Clinical Coordinator and work collaboratively to resolve the issues at hand.
  • Completes client psychosocial assessments and provides diagnoses; formulates a plan of treatment which promotes development and maintenance of individual goals designed to facilitate individual growth, improved social interaction, ability to manage mental illness and live independently.
  • Assesses and monitors condition of client to evaluate success of treatment and adapts interventions as needed.
  • Provides case management to include assessment of client/family needs, referral, links and support in accessing community services, psychiatrist, medical care, housing, rehabilitation programs, drug/alcohol services and social activity groups.
  • Monitors and maintains documentation of client services within client’s charts, ensuring compliance with Agency policies and procedures, Department of Mental Health contract and other related agencies’ standards and guidelines.
  • Develops and maintains therapist/interventionist skills through seminars, workshops, or other forms of training/education related to working with adults with a history of mental illness.
  • Attends and participates in program, Agency staff meetings; and related community agency meetings as required/requested.
  • Arrives to weekly individual/group supervision prepared with cases to discuss and ready to receive feedback and direction from supervisor.
  • Attend case conference as required.
  • Assist with facilitating program groups a minimum of one time per week.

REQUIREMENTS:

To perform effectively in this position, the employee in this position is required to have:

  • MFT, MSW, LCSW, LMFT or Licensed Psychologist required.
  • A working knowledge of conducting individual counseling sessions for those with a history of mental illness and their families.
  • Strong working knowledge of crisis intervention and individual counseling, documentation, psychiatric intern supervision, and multicultural and socio-economic issues.
  • Know and comply with Agency policies and procedures, HIPAA, Department of Mental Health, policies and documentation guidelines, and other state/federal regulations relating to service programs for adults with a history of mental illness.
  • Basic advocacy skills.
  • Present ideas, information, and viewpoints clearly, both verbally and in writing.
  • Efficiently use the personal computer to include a variety of word processing software.
  • Utilize analysis, experience, and judgment to make decisions.
  • Demonstrate commitment to team objectives and SHIELDS For Families philosophies.
  • Adapt to changing needs by acquiring new skills and knowledge.
  • Current California driver’s license and a driving record acceptable to the Agency’s insurance carrier.
  • Provides services within a culturally sensitive, ethnic informed model of practice and is respectful to the community and clients.

These requirements are typically met by a combination of education at a Master’s level and experience that includes two years’ experience working with individuals with a history of mental illness in a social service/mental health environment.

We provide counseling services for clients with primary addictive behavior problems, and associated mental health problems.
