Asian American Recovery Services (AARS), a program of HealthRIGHT 360 is looking for a committed, client-centered, full-time Care Coordinator III who is passionate about serving clients whose mental health needs range from mild-moderate to severely mentally ill in an outpatient setting. The Care Coordinator III provides mental health and co-occurring individual and group therapy to youth based on individualized assessments. Intensity of services is based on the client’s stage of change and response to treatment. Care Coordinator III’s provides after-hours crisis line support on a rotating basis. BBS supervision is available. Hours of operation are Mondays and Fridays from 9:00AM to 6:00pm; Tuesdays, Wednesdays, and Thursdays from 9:00AM to 8:00PM.
Individual Treatment Responsibilities: Facilitates intakes, comprehensive assessments, individualized treatment planning, crisis, case management and individual counseling sessions with clients. Provides learning experience opportunities and offers clinical support to assist clients in meeting their treatment goals. Proactively links clients to both internal and external resources based on their treatment needs and follows up on the progress/status.
Provides after-hours crisis line support on a rotating basis.
Treatment Setting Responsibilities: Facilitates group counseling and education sessions as appropriate. May conduct community outreach and education workshops. Performs crisis intervention and works with treatment team to stabilize crisis situations. Attends required trainings and meetings. Plans and facilitates client celebrations and special events. May be asked to work on a weekend or during after-hours for crisis stabilization, community education, resource fairs, and special events.
Documentation Responsibilities: Collaborates with each caseload client and other available internal and external resources to develop/maintain treatment plans, transition plans, progress notes and appropriate updates in support of the health and recovery needs of the client. Properly documents all individual and group counseling sessions, attempts to contact, and completes the discharge paperwork/process and required agency assessments in timely manner. Also, maintains accurate records by data entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.
Will participate in Peer Chart/ Quality Records Reviews for preparation of program audits. Will adhere to documentation policies and procedures set forth by funder and Compliance Department.
Be responsive and pro-active in all correspondences with referring parties, judges, probation and parole officers, social workers, and the Quality Improvement Dept. via treatment status reports, phone calls, faxes, etc. Will provide informed, written status reports to the above entities as appropriate.Will assist in providing written progress reports/narratives to Clinical Program Manager or Directors as requested.
