Program Overview – Adelante:

Located in Chula Vista, CA, we provide intensive, community-based case management services and housing for homeless adults throughout Southern San Diego County. We deliver fully-integrated services to clients who are homeless, diagnosed with a serious mental illness, as well as to individuals with co-occurring substance use and mental health disorders.

Our program utilizes the evidence-based practices of Assertive Community Treatment (ACT) Strength-Based Case management, and the Housing First model to support our clients in the higher level of community care and to assist them in reaching their goals: living a life that is not dominated by mental illness, homelessness, or substance use. Our teams are a model of care, providing treatment, rehabilitation, housing, and case management supports.

Typical Duties of the Mental Health Counselor – PSR Specialist

•Provides case management and mental health services for clients including coordinating and monitoring the activities of the individual treatment team; develops, writes, implements, evaluates, and revises overall treatment goals and plans in conjunction with the ACT treatment team
•Provide individual counseling and symptom management; educate and support clients’ families and advocate for clients’ rights
•Conduct comprehensive assessment of psychiatric mental status, diagnosis, and family structure and relationships; provide multiple-stage individual and group treatment to develop a trusting relationship with clients so that they may more freely discuss substance use and its effect on mental and physical health and daily functioning
•Document client progress and provide direct clinical services to clients on an individual, group, and family basis to teach behavioral symptom-management techniques to alleviate and manage symptoms

Required Skills and Qualifications

•Bachelor’s degree in a mental health-related field such as Social Work, Psychology, Counseling and licensed or registered in CA as IMF, ASW, PCCI, or waiver-eligible (ABD) psychologist candidate as required by contract
•Minimum of 2 years of experience working with a mental health population, especially with individuals with severe and persistent mental illness and co-occurring disorders is strongly preferred
•Knowledge of clinical and rehabilitation theory, knowledge of substance use disorders
•Ability to read, write, and speak fluently in Spanish; knowledge of Latino culture required
•A valid California driver’s license in good standing and eligibility to be an approved CRF driver is required to fulfill the essential functions of this position. In addition, proof of insurance is required prior to driving personal vehicles on company business.
