Telecare’s Outpatient Programs in Modesto are funded by Stanislaus County Department of Mental Health and the Mental Health Services Act (MHSA). The variety of options supports individuals in all levels of their recovery while offering choice and flexibility to members and their families.
POSITION OBJECTIVE
Using Psychiatric Rehabilitation principals and Recovery Philosophy, the Clinician provides clinical and administrative supervision, coaching and education for assigned team members with appropriate psychiatric support to ensure service excellence and courteous, helpful, and respectful services to customers
Provides direct customer, community support, crisis intervention and case management services to assigned customers
Provides education and support to customers and their families
Develops service plans in collaboration with other team members
Coordinates service needs and collaborates with other sservices and agencies such as Child Protective Services and Health and Mental Health Services
Completes assessments and determines the appropriate level of services for new customers
QUALIFICATIONS
Licensed or licensed waivered mental health professional (ACSW/ LCSW, MFTi/MFT, Psy.D/Ph.D) with two years of direct clinical service is required
Outpatient clinic or community mental health service is preferred
Experience in providing intensive case management and intensive community supports using psychiatric rehabilitation concepts is highly desirable
Applicant must be fingerprinted and receive clearance from the Department of Justice, Office of inspector General, and the DMV
Must have a valid driver’s license and proof of insurance
Must have completed first aid and CPR training or obtain certification within four weeks of hire
