Casa Pacifica is currently accepting applications for a Wraparound Family Facilitator in Santa Barbara County. Successful candidate will work out of the Santa Barbara office site but will travel throughout Santa Barbara County for client and/or program meetings. Candidate will receive supervised MFT Intern/MSW Intern hours towards BBS licensure.

Under the guidance of the Wraparound Program Manager & Clinical Supervisor, the Family Facilitator coordinates the development and implementation of family service plans (Plan of Care), and provides much of the case management services to children and their families enrolled in Wraparound Services.

Duties include:

  • Provide leadership in the development, implementation, and evaluation of individual wraparound plans.
  • Assume responsibility for guiding, leading, and facilitating child and family teams.
  • Coordinate, schedule, and facilitate child and family team meetings for development of individualized wraparound Plan of Care.
  • Assume responsibility for the Family Plan of Care management activities and implementation of services / coordination of resources for children and families.
  • Assist in monitoring the Child and Family Plan expenses / budget and secure approval for overall budgeted plans.
  • Monitor and support the provision of unconditional care by reassessing and “redoing” individual plans as needed, ensuring that families have access to resources, and maintaining effective relationships with child and family team members.
  • Provide crisis management to families and child / family teams when plans disrupt and onsite consultation to others involved in plans.
  • Participate in a rotating 24 hour on-call system.
  • Assist in the preparation of quarterly and annual service reports.
  • Ensures timely and appropriate communication and collaboration with partner agencies with community resources.
  • Assume responsibility for ongoing contact with referring agencies for every case and document contact.
  • Carry a case load averaging eight to ten families.
  • Remain current and accurate with billing and meet target goals.
  • Monitor and ensure effective linkages with community and agency service providers.
  • Maintain clinical oversight of the Child and Family Team (CFT) meetings.
  • Establish and maintain positive and effective working relationships with internal staff and external resources.

Qualifications: Master’s degree is required w/ at least two years experience working with SED/high risk children, adolescents, and their families with in a mental health setting. Must be a Marriage & Family Therapy (MFT) Intern, Licensed Professional Clinical Counselor (LPCC) Intern or Masters Social Work (MSW) Intern. Qualified candidates must have strong case management/facilitation skills and the ability to engage parents as partners in the treatment of their child. Individual must also demonstrate leadership qualities and have the ability to successfully work with families from diverse ethnic and cultural groups. Candidate must also and have experience with crisis management. Bilingual preferred.
