Position: Clinical Case Manager III
Salary: $43.46 per hour (Non-Exempt), annualized at $90,400.27 with employer-covered health benefits
Schedule: Monday to Friday 9am to 5pm with 1 hour paid lunch break

Reports to: Clinical Program Director

This position is with Conard House, Inc. Outpatient Services within the Supportive Housing Program. Qualifications:  

  • Master’s degree in Psychology, Counseling, or closely-related field of concentration.
  • Registration with the CA Board of Behavioral Sciences required. (Must be licensed or waivered). Licensure hours for LCSW, MFT, LPCC or Psychology are available.
  • 2 years of experience in any occupation or position providing direct client services in a mental health setting preferred.
  • Experience in mental health services, including psychiatric diagnostic assessment, individual and group counseling/therapy, psychosocial case management, and crisis intervention.
  • Demonstrated effective advocacy, organizational, and communication skills.
  • Experience working with diverse populations and diverse teams.

Responsibilities: 

  • Provide individual and group psychotherapy, psychosocial case management, rehabilitation services, diagnostic assessments, plan development, and crisis intervention. Ensure all services are documented in Epic in accordance with Conard House documentation guidelines.
  • Conduct psychiatric assessments to determine medical necessity and clinical appropriateness for program eligibility.
  • Offer residents support and resources on a wide range of concerns in a constructive, non-judgmental manner.
  • Facilitate the admission and orientation process for new residents.
  • Assess residents’ needs for medical and psychiatric evaluations and coordinate appropriate referrals.
  • Develop and implement case management plans, money management strategies, and contractual agreements to support resident stability.
  • Assist residents in developing essential daily living skills.
  • Advocate on behalf of residents to secure entitlements, healthcare, mental health services, clothing, and other necessary resources. Ensure timely and accurate completion of applications and forms.
  • Lead therapeutic groups and structured activities aimed at fostering independent, stable living.
  • Meet established monthly billing targets.
  • Encourage community engagement by fostering an inclusive and supportive living environment where residents can interact and socialize.
  • Respond to crisis situations in accordance with organizational procedures and promptly consult with a supervisor as necessary.
  • Develop and implement discharge plans to ensure smooth transitions for residents exiting the program.
