Position Title: Mental Health Clinician (Bilingual)
Program: Positive Solutions (Adult/Older Adult Mental Health Services) – North County (Both Activity Road and San Marcos Locations)
Classification: Non-Exempt, Full Time
Salary: $20.00 – $22.00 per hour; Commensurate with Experience
Summary and Range of Authority: The Positive Solutions Program is funded by the County of San Diego, Mental Health Services Act – Prevention and Early Intervention (PEI). The program focus is to provide outreach, mental health prevention, and early intervention to homebound isolated seniors residing in North County and Downtown San Diego. Under the direction of the Program Manager, the Mental Health Clinician will provide short-term evidence-based treatment to homebound seniors and socially isolated older adults who are at risk for depression or other mental health related issues.
Mental Health Clinicians provide short-term evidence-based treatment (PEARLS) and some case management to homebound seniors who are showing symptoms of depression or other related issues. All clinical services are provided in the senior’s place of residence. As needed, Mental Health Clinicians may be paired with meal delivery drivers to conduct mental health screening on same day that meals are delivered. Essential Duties and Responsibilities:
Functions as a member of a multi-disciplinary team and participates in diagnosis, treatment, discharge planning and peer review.
Prepares medical records of all services rendered including PEARLS paperwork, psychiatric history, mental status examinations and progress notes, treatment plans, quality assurance, etc.
Prepares timely and accurate documentation of all clinical services rendered
Receives referrals from providers and community members with appropriate follow-up including crisis intervention and short-term counseling.
Performs outreach by pairing with outreach worker to conduct screening and provide psycho-education to seniors.
Performs intake assessments for clients in need of short term treatment, such as PEARLS.
Work collaboratively with Case Manager and Senior Community Worker to assist clients in obtaining SSI/MediCal/Medicare or other benefits and community resources.
Will agree to use the PEARLS model of treatment as the primary modality of treatment in the client’s residence or areas where seniors congregate.
Clinician will provide some outreach and education to Meal Delivery drivers and other community partners so that they can assist in identifying potential clients who may be at risk for depression.
Coordinate and conduct joint home visits with Home-Delivery-Meal staff and other potential partners.
May refer clients to contracted mental health providers or similar providers for long-term psychiatric care and medication management.
Develop relationships with ancillary providers from Senior Center and other senior providers.
Participates in staff meetings, case conferences, training, treatment utilization reviews and other quality assurance activities.
Must be comfortable doing in-home visits and willing to travel daily to see clients
Provide training and guidance to interns as needed or as requested
Attends Network Meetings or other Meetings as assigned:
Maintain appropriate productivity (60% Billable Time) which includes direct clinical service time, outreach time, documentation time and travel time.
May attend County Meetings, outreach, other community meetings/trainings, and/or Peer Chart Review meetings in place of Program Manager as requested.
Must be able to travel between Central Region and North Region of San Diego County to provide clinical services as needed.

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Spends approximately 70% of work time sitting and meeting with others or working at a desk and/or computer. Spends approximately 30% of work time standing or walking within the work area or outside the office at meetings and events, also bends, twists, stoops and reaches. Ability to communicate in writing and verbally. Regularly required to sit and talk or hear. Frequently is required to use hands to touch, handle or feel and reach with hands and arms. Occasionally lifts and/or moves up to 25 pounds. Working conditions are normal for an office environment, event venues and a commercial kitchen. The noise level is usually moderate.
